Eureka Community Health & Wellness Center is Open Door Community Health Centers’ (ODCHC) flagship clinic, with construction being completed in 2012. It is also our largest clinic. Located in Eureka, California, the most populous area of Humboldt County, it is convenient to most residents of Humboldt County. This clinic is designed to support an integrated, team-based approach to primary care, including family medicine, pediatrics, and behavioral health. This site is also the core of the ODCHC pediatrics program. In addition to traditional appointments, the clinic offers evening and weekend same-day visits for pediatric care. This site is home to our FNP/PA Residency Program offered in collaboration with the Weitzman Institute, Community Health Centers, Inc. of Connecticut. The Pediatric program is home to three Pediatricians as well as three Pediatric Advance Practice Clinicians. ODCHC uses the EPIC electronic health record system and has teleconference and telehealth capacity in all sites.
ODCHC qualifies for National and State Loan Repayment Programs offering up to $50,000 for a two year service obligation. ODCHC Medical HPSA score is 17.
